Paperwork Required Prior To Scheduling ABA Intake
For Returning ABA and Adult Clients
The following paperwork is needed to be emailed to clientservices@sonnenbergllc.com in order to request services for returning clients:

Copy of Primary insurance card, front and back

Secondary insurance card (if applicable), front and back
​
​Once you have requested services, we will reach out to you and may request additional paperwork be sent via email or fax.​
​We will reach out to you within 1-2 weeks after we receive the request.
​If you are having problems with any of the forms or if you have any questions please email the Client Services Department at clientservices@sonnenbergllc.com.
